T&Cs for Lloyds Banking Group 'Win Free Treatment' competition

This offer is open to all Lloyds Banking Group colleagues who visit www.ultralase.com/landing/57 and enter their name, address and contact information.

1. The Ultralase Lloyds Banking Group Prize Draw will take place on Friday 8th January 2010.
2. The closing date for entries will be Thursday 31st Dec 2009.
3. The winner is entitled to free laser eye surgery, worth up to (£2250 per eye).
4. Treatment is subject to a free consultation at which suitability is assessed and the most appropriate vision correction treatment type recommended.
5. Even though Ultralase has such a wide range of vision correction treatments, please note that around 10% of those attending consultations are deemed unsuitable for treatment.
6. If the prize winner is not deemed suitable for vision correction treatment, the prize is not transferrable.
7. The surgeon's decision whether to proceed is final.
8. There is no cash alternative.
9. Those who have already had vision correction treatment will not be entered into the draw.
10. Open only to UK residents.
11. Entrants must be over 18 years of age.
12. The winner will be notified by telephone and / or post by Friday 18th December 2009.
13. The winner will be required to provide a photograph, testimonial and answer questions about their treatment which Ultralase will be permitted to publish in marketing material.
